The forms needed to apply for an import licence are available here.

The forms needed to apply for an import licence are available for download here (just click on the form's title). As an original signature is needed or in some cases original supporting documents, it is not possible to return applications electronically to ILB.

Form ILB/W:

This is a general application form and can be used to apply for all BIS import licences, i.e. certain textiles originating in Belarus, North Korea, certain iron and  steel products and firearms and ammunition. The form includes two pages the first of which is the application itself and the second page contains the guidance notes and general information about applying for import licences.

Form LA1:

This is the nomination of someone to sign applications on behalf of Directors, Partners or Proprietors. The person being authorised can work either in the importing company or their shipping agents.

Note: to authorise someone to sign on behalf of a limited company the form should be signed by a member of the board of directors of the limited company.

Form ILB/5: (DOC, 219 Kb) 

This is used to replace a paper import licence which has been lost or destroyed.

The forms can be downloaded but it is still necessary to print the form, sign it and forward it by post or courier to ILB. Currently it is not possible to send applications over the internet or by e-mail. Faxed applications can only be submitted for iron and steel goods by prior agreement with ILB.

It should be noted that this Department retains Crown copyright both on the printed and electronic versions of the forms. Any changes made to the forms remain the copyright work of BIS. It should be noted that permission to use the forms in any format is non-exclusive.
